SmartCOPD, formerly known as EmmaCOPD, is a validated intervention that can reduce hospitalization days by as much as 79%. This care module involves an implementation of SmartCOPD in which different healthcare providers within the region work together around the patient. The SmartCOPD intervention consists of a combination of the Digital Lung Attack Action Plan (DigiLAP) and the Activity Coach. SmartCOPD helps patients adopt optimal movement patterns and to independently recognize lung attacks and take appropriate action immediately when necessary. The SmartCOPD regional care module is designed to help first and second lines work better with each other and make referrals easier. In 2018-2020, LUMC conducted research on the effectiveness of SmartCOPD in patients who had two or more exacerbations with hospitalization per year prior to the study. The results of this study show statistically significant evidence that SmartCOPD reduces hospitalization days by 79% in this target group. The results were published in the Journal of Medical Internet Research (JMIR) in April 2021. Through the SmartCOPD regional care module, Viduet enables hospital, family physician, home care, physical therapists, and medical direction centers to work together around the patient. Viduet makes it easy to appropriately engage each involved healthcare provider in the patient’s care pathway. First, all involved caregivers are easily added to the patient’s community. Then, with a single button-press in the personal care program, caregivers can prepare a complete care path, including the desired settings for handling signals, for a new patient. In this way, the patient’s care path can also be easily converted at any time. This allows patients who are referred or referred back to be easily transitioned to their new care pathway.

This care module can be used in addition to other COVID-19 care modules. Through this care module, Viduet offers the ability to monitor patients who are monitored at home while also being offered oxygen at home. Within this care module, patients receiving home oxygen from Vivisol are monitored by Vivisol nurse dispatchers. In that case, they not only monitor for symptoms appropriate to COVID, but also monitor the patient’s oxygen use. Through questionnaires, patients report their oxygen use in the Viduet app. Viduet brings oxygen use trends to Vivisol, hospital caregivers and the patient themselves. Insights from these readings can be used by health care providers to optimize treatment. Changes in oxygen consumption are noted by Vivisol and are compared with measured oxygen saturation values. When thresholds are exceeded, Vivisol contacts the patient’s healthcare provider. In this way, patients receiving oxygen can be monitored safely at home. Finally, through Viduet’s home monitoring, Vivisol can also remotely support the safe withdrawal of oxygen and any medication.

This care module enables a better understanding of the rehabilitation process of Long COVID patients and supports patients with Long COVID during their recovery. This care module uses questionnaires to monitor progress and any complaints. Viduet makes trends in symptoms progress insightful for both patient and caregiver. In this way, caregiver and patient gain a better understanding of the rehabilitation process and a person-centered treatment plan can be determined. Since optimal movement patterns can greatly enhance the rehabilitation process of Long COVID patients, it is recommended to combine this module with the use of the Activity Coach.

Patients measure at agreed-upon measurement times and easily sync their measurements in the Viduet app. For monitoring for symptoms, the patient completes daily questionnaires. Once thresholds for symptoms or readings are exceeded, patients and the appropriate healthcare provider are immediately alerted in the appropriate manner. In this way, immediate and appropriate action can be taken as soon as the situation calls for it. Through the care module COVID-19 regional, Viduet enables hospital, family physician, emergency room, home care, and medical direction centers to work together around the patient.
