This module enables monitoring of patients with hypertension. The patient takes a protocolized home blood pressure measurement at the agreed upon times according to the Dutch CVRM guideline. This involves using the validated Microlife blood pressure monitor, which in addition to measuring blood pressure also screens for atrial fibrillation. In addition, questionnaires can be used to provide insight into trends in disease progression for caregiver and patient. When readings exceed thresholds, caregiver and patient are alerted in the desired manner. This way, appropriate action can be taken immediately.

This module can be used when a patient's medication needs to be set. During this period, blood pressure medication is often tapered off and later rebuilt. The patient measures his blood pressure twice each day throughout the period. This involves using our protocolized home blood pressure measurement in which patients measure their blood pressure entirely according to the Dutch hypertension guideline. When readings exceed thresholds, caregiver and patient are alerted in the desired manner. This way, appropriate action can be taken immediately.

This care module can be used for patients with hypertension who need further investigation into the underlying cause of their hypertension. Using protocolled home blood pressure measurements that are fully compliant with Dutch and European hypertension guidelines, Viduet offers the healthcare provider an alternative to conventional 24-hour measurement. In addition, this care module provides the ability to query symptoms at the appropriate time through questionnaires that can be triggered based on blood pressure readings. Finally, this care module helps set up the care pathway more efficiently by actively reminding patients of all their checkup appointments, such as getting blood drawn. When using our protocolized home weekly measurement, patients measure their blood pressure at home in a completely reliable manner. This gives you as a healthcare provider a better understanding of your patients' blood pressure data. Home blood pressure measurement uses the protocolized home measurement advised by the Dutch CVRM guideline. This care module targets patients with complex hypertension. Protocolized home blood pressure monitoring is used monthly in this care module to monitor patients for hypertension. In addition, questionnaires can be used to better identify trends in symptoms. Based on these measurements, the health care provider can determine how often the patient should visit the hospital for a consultation. Also, based on these insights, the health care provider can determine if medication needs to be adjusted. The Hypertension medication titration module can then be used temporarily for this purpose. Exercise is an important factor that can influence patients' disease course. Using the Activity Coach care module, you ensure that the patient is coached to adopt optimal movement patterns. Thus, the patient is coached, without overexerting himself, to still move enough to promote the rehabilitation process. When you choose to deploy the Activity Coach, the patient will track their movement patterns through the smartwatch. Using our specially developed software for this smartwatch, the patient will be coached throughout the day to move within the ranges set by their healthcare provider, even if the patient moves too much. This care module can easily be combined with coaching by a physical therapist. In that case, the physical therapist can set the optimal bandwidths and adjust them as needed. Through a smart algorithm, Viduet recognizes when the patient moves out of bandwidth for an extended period of time. Viduet can then also inform the physical therapist and patient in the desired manner, so that the set bandwidths can be revised if necessary.
