SmartCOPD, formerly known as EmmaCOPD, is a validated intervention that can reduce hospitalization days by as much as 79%. This care module involves an implementation of SmartCOPD in which different healthcare providers within the region work together around the patient. The SmartCOPD intervention consists of a combination of the Digital Lung Attack Action Plan (DigiLAP) and the Activity Coach. SmartCOPD helps patients adopt optimal movement patterns and to independently recognize lung attacks and take appropriate action immediately when necessary. The SmartCOPD regional care module is designed to help first and second lines work better with each other and make referrals easier. In 2018-2020, LUMC conducted research on the effectiveness of SmartCOPD in patients who had two or more exacerbations with hospitalization per year prior to the study. The results of this study show statistically significant evidence that SmartCOPD reduces hospitalization days by 79% in this target group. The results were published in the Journal of Medical Internet Research (JMIR) in April 2021. Through the SmartCOPD regional care module, Viduet enables hospital, family physician, home care, physical therapists, and medical direction centers to work together around the patient. Viduet makes it easy to appropriately engage each involved healthcare provider in the patient's care pathway. First, all involved caregivers are easily added to the patient's community. Then, with a single button-press in the personal care program, caregivers can prepare a complete care path, including the desired settings for handling signals, for a new patient. In this way, the patient's care path can also be easily converted at any time. This allows patients who are referred or referred back to be easily transitioned to their new care pathway.

With this care module, Viduet offers the ability to use home monitoring to reduce hospital stays for patients with COVID-19 or keep patients out of the hospital at the front end.   Patients are monitored at home for oxygen saturation, body temperature, respiratory rate and relevant symptoms. If desired, Viduet also offers the ability to monitor patients for glucose day curves. To perform all these measurements, the patient receives Viduet-connected measuring devices. Patients measure at agreed-upon measurement times and easily sync their measurements in the Viduet app. For monitoring for symptoms, the patient completes daily questionnaires.   Once thresholds for symptoms or readings are exceeded, patients and the appropriate healthcare provider are immediately alerted in the appropriate manner. Exercise is an important factor that can influence patients' disease course. Using the Activity Coach care module, you ensure that the patient is coached to adopt optimal movement patterns. Thus, the patient is coached, without overexerting himself, to still move enough to promote the rehabilitation process. When you choose to deploy the Activity Coach, the patient will track their movement patterns through the smartwatch. Using our specially developed software for this smartwatch, the patient will be coached throughout the day to move within the ranges set by their healthcare provider, even if the patient moves too much. This care module can easily be combined with coaching by a physical therapist. In that case, the physical therapist can set the optimal bandwidths and adjust them as needed. Through a smart algorithm, Viduet recognizes when the patient moves out of bandwidth for an extended period of time. Viduet can then also inform the physical therapist and patient in the desired manner, so that the set bandwidths can be revised if necessary.
