Using our specially developed software for this smartwatch, the patient will be coached throughout the day to move within the ranges set by their healthcare provider, even if the patient moves too much. This care module can easily be combined with coaching by a physical therapist. In that case, the physical therapist can set the optimal bandwidths and adjust them as needed. Through a smart algorithm, Viduet recognizes when the patient moves out of bandwidth for an extended period of time. The results were published in the Journal of Medical Internet Research (JMIR) in April 2021. If desired, the validated SmartCOPD intervention can also be used within primary care. For patients with a low disease burden, Viduet also offers a special configuration of the DigiLAP within this care module. How does it work?
The patient receives a blood pressure measurement package from Viduet. Viduet checks and calibrates the measuring equipment and makes sure it is delivered to the patient. The patient engages in multi-day protoccluded blood pressure measurement. Viduet provides a guide for this and can assist if needed.
Viduet can set signals at certain readings, if the caregiver so desires. Thus, the caregiver or caregiver can be alerted to an extremely high reading, or to any reading the caregiver desires.
Also, for a given value, the patient can be directed to the home health website to read more.
The measurement results are automatically put into a clear report, which can be viewed directly on the Viduet dashboard. In addition, results of the protocolled weekly measurement can be sent immediately to the HIS as a lab value. All results can be viewed at any time, with insight into the trend over time.

Also, for a given value, the patient can be directed to the home health website to read more.
The measurement results are automatically put into a clear report, which can be viewed directly on the Viduet dashboard. In addition, results of the protocolled weekly measurement can be sent immediately to the HIS as a lab value. All results can be viewed at any time, with insight into the trend over time.

SmartCOPD, formerly known as EmmaCOPD, is a validated intervention that can reduce hospitalization days by as much as 79%. This care module involves an implementation of SmartCOPD in which different healthcare providers within the region work together around the patient. The SmartCOPD intervention consists of a combination of the Digital Lung Attack Action Plan (DigiLAP) and the Activity Coach. SmartCOPD helps patients adopt optimal movement patterns and to independently recognize lung attacks and take appropriate action immediately when necessary. The SmartCOPD regional care module is designed to help first and second lines work better with each other and make referrals easier. In 2018-2020, LUMC conducted research on the effectiveness of SmartCOPD in patients who had two or more exacerbations with hospitalization per year prior to the study. The results of this study show statistically significant evidence that SmartCOPD reduces hospitalization days by 79% in this target group. The results were published in the Journal of Medical Internet Research (JMIR) in April 2021. Through the SmartCOPD regional care module, Viduet enables hospital, family physician, home care, physical therapists, and medical direction centers to work together around the patient. This is made possible in part by the architecture of the platform. The image below shows schematically, using the Viduet care rings model, how Viduet enables this collaboration. Viduet makes it easy to appropriately engage each involved healthcare provider in the patient's care pathway. First, all involved caregivers are easily added to the patient's community. Then, with a single button-press in the personal care program, caregivers can prepare a complete care path, including the desired settings for handling signals, for a new patient. In this way, the patient's care path can also be easily converted at any time. This allows patients who are referred or referred back to be easily transitioned to their new care pathway.
