With this care module, Viduet enables patients experiencing symptoms consistent with atrial fibrillation to perform ECG measurements themselves at the time they experience complaints. For this purpose, patients are provided with a 6-lead ECG device on loan. This compact device is connected to the Viduet app via Bluetooth. After each measurement, the ECG is immediately visible in Viduet for the healthcare provider. In this way, Viduet enables reliable diagnosis of atrial fibrillation.

With this care module, Viduet enables patients with heart failure to take greater control of managing their own health. Patients regularly measure their weight, blood pressure, and, if applicable, oxygen saturation. Through our connected measurement devices, all values are transmitted directly to Viduet.

As soon as a patient records values outside their defined ranges, a questionnaire is immediately sent to assess relevant symptoms.

Viduet supports patients in timely recognition of any deterioration. Depending on the severity of the decline, patients are directly coached to either adjust their medication or contact the hospital. The hospital can also be informed in the appropriate way, ensuring that important events are properly recorded.

With the **Heart Failure Regional** module, different healthcare providers within the region can better align the care they deliver to patients with heart failure. This care module enables Viduet to remotely monitor patients with heart failure. Where possible, patients are given greater control over the self-management of their health. Through home measurements, Viduet provides better insight into disease progression and enables immediate action in case of deterioration.

Within the Heart Failure Regional module, patients regularly measure their weight, blood pressure, and, if applicable, oxygen saturation at home. The patient also regularly completes questionnaires. Measurements are taken using devices connected to Viduet. Patients measure at agreed times and easily synchronise their readings in the Viduet app. As soon as predefined thresholds for symptoms or measurements are exceeded, both patients and healthcare providers are immediately alerted in the appropriate way. This allows for timely intervention when the situation requires it.

Through the Heart Failure Regional care module, Viduet facilitates collaboration between hospitals, general practitioners, home care services, physiotherapists, and medical coordination centres around the patient. This is made possible by the platform’s architecture. The diagram below illustrates, using the Viduet care ring model, how this collaboration is structured.

Viduet makes it easy to involve each healthcare provider in the patient’s care pathway in the appropriate way. First, all involved providers are easily added to the patient’s community. Then, with a single click, healthcare providers can configure a complete care pathway in the personalised care programme, including the desired settings for handling alerts for a new patient. In this way, the patient’s care pathway can be easily adjusted at any time. This enables smooth transitions for patients who are referred or transferred to a different care pathway.

With this care module, Viduet enables intensive monitoring of patients with heart failure, focusing on weight gain, blood pressure, symptoms, and, if desired, oxygen saturation.

Patients regularly measure their weight, blood pressure, and, if applicable, oxygen saturation at home. Through our connected measurement devices, all values are transmitted directly to Viduet. Patients also complete questionnaires on a regular basis.

As soon as the measurements indicate that action is required, the hospital and/or patient is immediately informed in the appropriate way. This enables timely intervention and reduces the administrative burden of recording these values.

Patients are directly coached using the action plan. This action plan is created by the healthcare provider and defines four phases (green/yellow/orange/red). Patients receive reliable measurement equipment from Viduet, which has been pre-checked and calibrated. Patients perform regular home measurements, such as weight and blood pressure. In addition, they regularly complete questionnaires via the app and can always access the heart failure action plan.

The algorithm determines which phase of the heart failure action plan the patient is in. This is based on combined trends in weight measurements, blood pressure, heart rate, and reported symptoms. Patients are then directly coached using the action plan, depending on the phase they are in. This encourages them to continue as normal (green), prompts early action (yellow), advises them to contact their healthcare provider (orange), or to call emergency services in urgent situations (red).

Exercise is an important factor that can influence patients’ disease course. Using the Activity Coach care module, you ensure that the patient is coached to adopt optimal movement patterns. Thus, the patient is coached, without overexerting himself, to still move enough to promote the rehabilitation process.

When you choose to deploy the Activity Coach, the patient will track their movement patterns through the smartwatch. Using our specially developed software for this smartwatch, the patient will be coached throughout the day to move within the ranges set by their healthcare provider, even if the patient moves too much. This care module can easily be combined with coaching by a physical therapist. In that case, the physical therapist can set the optimal bandwidths and adjust them as needed.

Through a smart algorithm, Viduet recognizes when the patient moves out of bandwidth for an extended period of time. Viduet can then also inform the physical therapist and patient in the desired manner, so that the set bandwidths can be revised if necessary.
